Purpose
You use this report to generate the following forms that are related toemployment insurance:

• Qualification Acquisition Form

This form is for the employees who are qualified for the employmentinsurance. In these employees' records of the SI Basic Data JPinfotype (0140), subtype 4, the value of the Acquired/Lost fieldis 1 Acquired.
You generate this form by selecting the Acquisition noticeindicator on the selection screen.

• Moving-Out, In Notification Form

When the personal area or the personal sub-area of an employee changes,the insurance office changes accordingly. In their records of the SIBasic Data JP infotype (0140), subtype 4, the value of theAcquired/Lost field is 3 Office Change.
You generate this form for these employees in the above situation byselecting the Moving-out, in notice indicator on the selectionscreen.

• Qualification Loss Form

When an employee retires, he or she is not qualified for employmentinsurance any more. In their records of the SI Basic Data JPinfotype (0140), subtype 4, the value of the Acquired/Lost fieldis 2 Lost.
You generate this form for the retired employees by selecting theLost notice indicator on the selection screen.
Note:
If the employee's leaving day is earlier than October 1, 2007, thereport displays the employee's employment insurance data in the previousone year till leaving day
If the employee's leaving day is later than October 1, 2007, the reportdisplays employee's employment insurance date in the previous two yearstill leaving day
You can generate the Unemployment Insurance Resignation Certificate
after creating this form. To display the Qualification Loss Form
and the Employment Insurance Certificate, choose Lossnotice + Certification option on the result screen.
The Unemployment Insurance Resignation Certificate applies to theemployees with the following insurance deduction status:
Ippan
Ippan(Menjo)
Exec.Work
Exec.Work(Menjo)
Tanki
Kounenreikeizoku
Exec.(Kounenreikeizoku)
Note:
As of October 1, 2007, for the employees with the following insurancededuction types, the report displays the data as general insured personsin the certificate:
Tanjikan
Tanjikan(Menjo)
Tanjikan(Kounenreikeizoku)
