Fixed-Term Contract Statement for Standby/Stand-In Workers.

Purpose
The fixed-term contract statement is used to periodically report thenumber of days worked by standby workers. By choice, the number of daysworked are reported on a quarterly basis. The form must be signed bythe insured and sent to the health insurance fund [ziekenfonds]. If theinsured was also privately insured for the days worked it is possibleto apply to the health insurance fund for a reimbursement of theemployee's contribution.
