Description
Program that generates the form with the 'Quarterly Company INCESContribution'.
The report is executed according to the selection parameters: period andcompany.

Precondition
You must indicate the companies for which you want to generate the form.

Output
The form shows the total contribution amount by employee group andsubgroup, detailing the amounts and contributions corresponding to wagetypes that form the INCES contribution assessment basis.
